New
Patient Procedures :
|
On
the day of your first visit, you will need to bring with you all
the completed forms (listed below), your primary care physician
referral form if appropriate, your insurance cards, any x-rays,
scans or pertinent lab work you have had for the condition and
any medical records (history of present illness) from your
current physician.
Any
patient who comes in for an appointment without their referral
(should their insurance require one) will have to be rescheduled
to another date and time.
You
will need to be prepared to pay your insurance deductible and
your patient co-payment amount on the day of your visit. Please
contact your insurance carrier if you are unsure of your
benefits.

Refill
Policy :
|
Prescriptions may be refilled during business
hours Monday thru Friday only. The patient records are not
available once the office is closed, therefore refills cannot be
given on weekends. We may refuse to refill medications if we
have not seen you in six months or if you have missed several
appointments.
