Patient Services : Scope of Practice Insurance Information  New Patient Forms and Procedures
Insurance Information :                                   (See Plan Participation Listed Below)           We participate in most insurance plans. Some plans require referral from your primary physician. We are happy to assist you with this, and to file insurance claims for the convenience of our patients. Participating and non-participating insurance plans are listed below. Please contact the Billing Office  at (502) 894-9753 if you are not sure if we participate with your insurance company, or if you have other questions. It is possible for us to see patients with coverage from a non-participating plan under the following conditions: Patient must be told that there is a possibility that their insurance "may not" cover their office visit for going out-of-network. Patient must be told by Kentuckiana ENT that in seeing an out-of-network provider they will incur a much greater out-of-pocket expense.  Patient must have a referral from their primary care physician at the time of their visit  which has been authorized through the patient's insurance carrier.        Exceptions:          Aetna HMO - which can come straight from the PCP).         Healthsource - no referral, no payment.                 Non-Participating Insurance Plans :                                                                                      Top           GREAT WEST  (must call insurance company with Kentuckiana ENT tax id number to determine if your plan is  covered).     SHIO     TriCare (Champus)                 Participating Insurance Plans :     (  *  =  Referral Required )
